Test requirements and consent

Before attending a collection centre, I have checked that my doctor has signed the request form, I have read the patient consent section and signed the form, and I am at least 10 weeks pregnant.

By selecting this test, I consent to pay the correct amount for the test option selected by my doctor as indicated on the Generation NIPT request form

I understand I need to attend one of the 2000 + Healius collection centres. An administration fee of $25 will apply for a cancelled test where I have a change of mind or I visit a non-Healius site in error.

I understand this test is only collected on a Monday and I will need to attend a collection centre on this day. If I am located in regional WA or NT, I will need to do so before 11am.
