Test requirements and consent

I will secure a test request form signed by my doctor for PGx testing and return it to Genomic Diagnostics via the link sent in the email from us, before the kit will be sent to me.

The PGx Multi does not cover all medications in use. I have reviewed the medications list on Pharmacogenomics test page to ensure that it is relevant for me.
